This document designates an individual to serve as your patient advocate. This person can access your medical records and achieve your treatment choices in the event your physical or mental condition makes you unable to make such decisions. Also, with your express permission your Patient Advocate can carry out your decision to allow for the withdrawal of life support, including nutrition and hydration.
